What we do
A 90–120 minute review of your medical history, medications, specialists, and goals - resulting in a baseline care plan your whole family can understand.
We prepare questions, review labs, and attend appointments with you - taking detailed notes and making sure your concerns are actually addressed.
After every visit: diagnosis updates, medication changes, tests ordered, follow-ups, and action items - shared with you and authorized family members.
A continuously updated medication profile - what you take, why, who prescribed it, and what to watch for - with pharmacist-level review for interactions and duplications.
We keep your primary care physician, specialists, pharmacies, home health, and therapists on the same page, so recommendations don't conflict.
We provide personalized, in-home medication organization services tailored to your needs.
What we don't do
True North Rx works alongside your physicians - never in place of them. Being clear about where our role ends is part of how we keep you safe.
Why True North Rx
Most patient advocates can drive you to an appointment. Very few can review a 14-medication regimen, spot a risky duplication, and raise it with your cardiologist in their own language. True North Rx is led by a board-certified clinical pharmacist - so your navigator understands the medicine, not just the logistics.

Ongoing navigation and medication management - an always-current medication profile, coordination among your providers, and regular family communication. Includes one physician visit each month.

For any additional physician or in-home visits - the same preparation, clinical advocacy, and written follow-through, as often as you need it.

First year
Onboarding plus twelve months of subscription support, paid up front - $4,300 per year thereafter.
Services may be FSA/HSA eligible. Coverage varies by plan - check with your plan administrator.

Rachel is a residency trained, Board Certified Pharmacotherapy Specialist with years of clinical experience in medication management, patient education, and interdisciplinary care. She founded True North Rx because she saw the same story again and again: capable, caring families overwhelmed by a healthcare system that expects patients to coordinate their own care.
Her mission is simple - to be the trusted clinical partner who organizes, coordinates, and simplifies healthcare, so every appointment leads to better outcomes.
